Important - Do not submit any worksheets that the Office of Financial Aid has not requested. Check your additional information request list.
Return your completed worksheets to Financial Aid in one of three ways:

- fax to (815) 455-8594
- leave in the drop box outside Building A, Atrium Entrance.
Do not upload Verification Worksheets or Statements of Educational Purpose. The Office of Financial Aid requires the original paper copies to be delivered either in person or via mail.
